Appointment Instructions

How to make your appointment

To set up an appointment, just call Summit Family Planning at (800) 608-2630. You are welcome to call us with any questions you may have prior to actually scheduling your procedure.

Instructions for your appointment

Do not eat, drink or smoke anything for six (6) hours before your scheduled appointment. This includes candy, gum and water.
Do not use drugs or alcohol for 24 hours prior to the procedure.
Bring extra underwear, 2 or 3 sanitary pads, and socks or slippers.
Plan to remove contact lenses or dentures before the procedure.
Do not wear jewelry or bring valuables to the clinic.
If you are having IV sedation (twilight sleep), you must bring someone to drive you home.
Bring only one adult with you to the clinic. Please do not bring children.
Please bring payment with you. We accept cash and credit cards. The person whose name is on the credit card must be present.
